Electrical stimulation and insulin tests used with bilateral selective vagotomy.

چکیده
Completeness of vagotomy has been assessed by use of the electrical stimulation test and the insulin test in 50 patients suffering from chronic duodenal ulceration. A positive result from either was taken as evidence of incomplete vagotomy. The electrical stimulation test provided such evidence more often than the insulin test.No early positive insulin test result was encountered in any vagotomy performed with the aid of the electrical stimulation test. The number of late positive insulin results increased as the postoperative interval lengthened. It is believed that the routine use of the electrical test in this study increased the incidence of adequate nerve section.

The influence of the individual surgeon and of the type of vagotomy upon the insulin test after vagotomy.
Six hundred and seventy-six insulin tests performed in the early postoperative period after vagotomy and drainage for duodenal ulcer were analysed by the criteria of Hollander. Consultants had done 364 vagotomies, of which 17.6% were shown to be incomplete. Registrars and senior registrars had done 312, of which 12.2% were incomplete. Thus, consultants may not be significantly better, on averag...
متن کاملEffects of truncal, selective, and highly selective vagotomy on glucose tolerance and insulin secretion in patients with duodenal ulcer. Part I-Effect of vagotomy on response to oral glucose.
An oral glucose tolerance test was performed in patients who had undergone truncal vagotomy and pyloroplasty, bilateral selective vagotomy and pyloroplasty, or highly selective vagotomy without a drainage procedure at least six months earlier. The results were compared with those from patients with chronic duodenal ulcer before operation. In all three groups of patients after vagotomy more rapi...
متن کاملThe effects of varying the extent of the vagotomy on the myoelectrical and motor activity of the stomach.
The effects of varying the extent of vagotomy on the myoelectrical and motor activity of the stomach have been successfully studied in 21 patients undergoing either truncal, selective, or highly selective vagotomy for the treatment of chronic duodenal ulceration. The mean percentage time that regular antral myoelectrical activity was recorded preoperatively was 95.8% +/- 1.0 and this was decrea...
متن کاملAn evaluation of the Hollander test by graded vagotomy in the dog.
Hollander's insulin test was evaluated by performing successively partial denervation of the parietal cell area, highly selective, selective, and truncal vagotomy operations on three dogs. The rise in acidity, the timing of the highest rise in acidity, and the rise in acid output were examined. The mean rise in acidity (m-equiv/l) before vagotomy (106 +/- SE11, 120 +/- 23, 60 +/- 16) did not di...
متن کاملRecurrent ulceration after vagotomy and drainage with electrical stimulation test, 1957-69.
Between 1957 and 1969 700 patients with duodenal ulceration were treated by selective vagotomy and simple drainage using the electrical stimulation test to achieve complete nerve section. Ten of these patients have been re-admitted to the hospital with further ulceration, one with a lesser curve gastric ulcer and nine with recurrent duodenal disease. The first patient had gastric retention and ...
